Your Journey to Recovery Begins

When entering treatment with us, you will be assigned to a Bridge Team case manager and a primary case manager. The Bridge Team case manager will help you get familiar with the agency, set up an appointment with a Psychiatrist if needed, and help you determine your goals. You will work with the Bridge Team Case Manager on your most immediate service needs. The Bridge Team Case Manager will introduce you to your primary case manager on the Case Management Team. This Team will consist of a group of case managers who will assist you to fully develop the goals you set while working with the Bridge Team. One of the case managers on this Team will work with you closely. You may work with your case manager for as long as you need our services.

Your case management team works in the Community Support and Rehabilitation Department at Clermont Counseling Center. Your case management team’s mission is to provide support to people with a mental illness and other disadvantages. This support is termed “Role Recovery.”
