Your responsibilities
in the treatment process includes the following:
Participate
with your Case Manager and/or your Psychiatrist in planning your
treatment and/or rehabilitation. If you have an appointment at our
office, please arrive on time.
If you cannot
come, please call and cancel or reschedule.
Ask questions
if you don’t understand something.
Ask your Psychiatrist
questions if you don’t understand the medications you are
taking.
Show
respect for other consumers and providers.
If you
are able to pay a fee for service, please pay your bill on time.
(The agency will work with you to determine your ability to pay.)
If you have
a complaint or serious concern, please contact the agency’s
Client's Rights Officer. |
